Our expert team is dedicated to providing comprehensive care throughout your journey, from the initial evaluation to the follow-up visits. We understand the importance of a smooth transition, and that’s why we have a 24/7 clinical support team of Registered Nurses (RNs) and Medical Assistants (MAs) to assist you at all times. Our goal is to empower both patients and Primary Care Managers (PCMs) through remote patient monitoring (RPM) and constant support.
Communication is key, which is why we make it a priority to contact beneficiaries or caregivers within two business days following discharge, ensuring seamless continuity of care. Our providers conduct timely follow-up visits, typically within 7 or 14 days after discharge, depending on the complexity of medical decision-making involved.
We believe in educating our patients and keeping them informed. Through our Principal Care Management (PCM) program, you will receive valuable education and have the opportunity to sign or provide verbal consent. We have also streamlined the non-facility PCM process for your convenience.
In addition, we offer scheduled diagnostic testing and follow-ups, ensuring that all necessary procedures are taken care of promptly and efficiently. We are always striving for advancements in healthcare, and in the near future, we will be introducing RPM services to further enhance our offerings.
